Professional Services:

• Comprehensive skilled nursing assessments and ongoing reassessments

• Medication administration, monitoring, and management in accordance with physician orders

• Wound care, dressing changes, and monitoring of healing progress, chronic disease management, monitoring and education

• Post-hospital nursing follow-up and transitional care support

• Patient and caregiver education related to the client’s diagnosis, medications, treatments, and safety needs

• Coordination of care with physicians and other healthcare providers to support continuity of care

Initial Comprehensive Nursing Assessment: $150

Prices range from $95-$110 per visit or $85-$95 per hour

Care Coordination/Physician Communication (when ordered): $75 per occurrence
